Dialysis is an important daily process for individuals who have lost kidney functionality.
Although many medical advances have been made in the treatment of kidney and urinary tract disease, the emotional effects of kidney failure on patients and their families remain great because many areas of everyday living have to be changed.
Some important aspects for educating and supporting patients are:
- Creating awareness of pre-dialysis to patient and patient's family members and caregivers
- Prepare and help patients embark on a planned and structured dialysis programme
- Ease patients into their treatment regime and provide continuous support to them and their caregivers
- Care coordination to help patients schedule appointments, access resources and find support
- Active collaboration between nephrologists and care teams to provide holistic patient care, including comorbid condition management
Goals and Visions
Patient education is designed to support and enable patients to manage their life and optimise their health and well-being. The main goals are:
- Improved understanding of medical condition, diagnosis or disability.
- Understanding of methods and means to manage multiple aspects of medical condition.
- Improve trust between a patient and care provider through effective and clear communication.
- Predictive analytics for early patient identification, timely intervention, and proactive management of comorbid conditions to help delay CKD progression
